*All membership dues are non-refundable. Payments will be processed each month on the agreed upon date and will continue to be processed until notice of cancellation is provided, a minimm of 30 days prior next billing cycle. Memberships must be a minimum of 3 full consecutive months including but not limited to any possible pro-rated partial first mont in the event that my membership did not initially start on the 1st or 15th. Monthly Signature Treatments do not roll over or accumulate. If for whatever reason a Monthly Signature Treatment is not redeemed or used in full before the end of the billing cycle, it is forfeited. Only 1 Signature Treatment can be used per month and additional add on treaments can be purchased at the designated Membership level discount. Signature Membership discounts do not apply to prescription cost, program cost, Cellex-C products, Radiesse, Juvederm, Restylane, Sculptra, Perlane & Voluma.
